pre-eclampsia

General anaesthesia in women with pre-eclampsia:
  • Indication: patient unstable, altered level of consciousness, neurological deficits, contraindication for neuraxial anesthesia (1).
  • Particular attention and extreme vigilance should be given to ablating the hypertensive response to intubation, as this has been identified as a cause of direct maternal morbidity. Drugs that have been used for this purpose include alfentanil, fentanyl, remifentanil, MgSO4, lidocaine and esmolol. Care must also be taken to avoid complications on emergence from anaesthesia including hypertension, aspiration and acute pulmonary oedema (1).
  • Induction: Pentothal 5-7 mg/kg, Suxamethonium 1 mg/kg, Fentanyl 1-2 microg/kg (2).
